Check in form

All teams must check-in Friday May 2 at the Sleep Inn 1015 Oakvale Road Princeton WV from 5:00-8:00. There will be not late check-ins and check-ins will not be permitted on May 3. If unable to check in May 2 you can complete the online registration additional fees do apply.

Team rosters, medical releases and player pass cards are required for all teams and may be checked prior to games.
