Check-In Instructions

In-person check-in to take place 1 hour prior to your team's first Saturday game (players do not need to attend) at the venue HQ at the fields. Venue HQ will be set up at the tent located close to field #5 - Click HERE for a map of the fields.

Your team representative needs the following for successful check-in (scroll down for additional forms): 

For Current SSA Teams:

1. FIVE (5) copies of the tournament roster/game card (available below under 'Game Cards' once you have logged in using your SIC ID and password in the top right corner of this page), 1 for each game, plus 1 copy for the venue HQ staff to keep, listing all the players playing in the tournament (including any club pass/guest players*). To add players to your roster or delete players from it, please log-in to your SINC team account using the same user name and password as for the application and click on 'Event Tasks', then 'Quick Roster'. Once you have updated your roster, the player details will autopopulate your game card that you will print out below. At check-in, one copy will be retained by the tournament, while the other four will be returned to the team representative. The team representative will be responsible for handing one copy to the referees prior to each game. These cards will then be used by the referees as official game cards, after which the team representative of the winning team, or the home team in the event of a tie, will need to take the game card to the tournament HQ to allow us to update standings.

*A club pass player is a player who is registered to a Rec team within the same club other than the team he/she is playing for in the tournament, while a guest player is a player registered to a club other than the club to which the participating team belongs. A maximum of three club pass/guest players may be used for 7v7 teams and four for 9v9 teams, and the combined regular/club pass/guest players number of players on the team may not exceed the maximum roster size as listed in the rules. 

2. ONE (1) completed copy of the official SSA Tournament Waiver Form (either downloaded from the link below, or printed out from your own SIC team page linked to our tournament), signed by the parents of all the players who will be participating, including any club pass/guest players.

3. If you are an SSA team, and use any guest players (as defined above) in the tournament, you must also provide the player pass for each guest player as evidence that the player is registered with another youth soccer club as a Recreational player for the 22/23 seasonal year.

 

For Teams from Clubs Other Than SSA:

1. ONE (1) copy of its official state or US Club roster, listing players' names, DOB, and state registration ID #s. If you don't have this document already, please contact your league office (usually the registrar) to obtain a copy. We will keep this copy as proof of your roster and evidence that all of your players are properly registered and insured with the appropriate State or US Club soccer organization. If you have any club pass/guest players, please handwrite them into the bottom of the document, listing name, DOB, and unique state ID # (from player pass).

2. FIVE (5) copies of the tournament roster/game card (available below under 'Game Cards' once you have logged in using your SIC ID and password in the top right corner of this page), listing all the players playing in the tournament (including any guest players). To add players to your roster or delete players from it, pleas log-in using the same user name and password as for the application and click on 'Team Tasks'. Once stamped, one copy will be retained by the tournament, while the other four will be returned to the team manager. The team manager will be responsible for handing one copy to the referees prior to each game. These cards will then be used by the referees as official game cards, after which the coach/TM of the winning team, or the home team in the event of a tie, will need to take the game card to the tournament HQ to allow us to update standings.

3. Laminated state or US Club-issued player pass and a club Medical Release Form (does not need to be notarized) for each player on the roster. Please note that the Medical Release Form for the tournament is the same one provided to you by your own club for use during the regular-season as a means for you and/or the Assistant Coach/Team Manager to be able to provide one of your players with medical assistance should the player's parents not be present and for the parent to assume all financial obligations arising from that action. 

4. ONE (1) completed copy of the official SSA Tournament Waiver Form (either downloaded from the link below, or printed out from your own SIC team page linked to our tournament).

5. Out-of-State teams will need to provide a completed copy of their state association's 'Notification of Travel' form (or similar).

6. Please note that guest players will need to be listed on each copy of the tournament roster/game card. In addition, the player must have a valid state or US Club Soccer laminated player pass, the home club's Medical Release Form, and the parent/guardian must have completed and signed the team's waiver form. We will not need an official copy of the guest player's own team's state roster, nor will we need a birth certificate provided the player's state-issued or US Club Soccer-issued player pass has his/her DOB and unique player ID number on it.

7. If you have any club pass/guest players on your team, please add the players to your official game card/roster, handwrite the player's name and DOB (along with the player's unique state ID number) on your team's state roster and have the player's state player pass available for inspection at check-in.
