Check-In Documentation
1. A copy of its official Team Roster (printed out from below), plus proof of age for all players rostered to play.
If your players are registered for the 2018/19 or 2019/20 seasons with a youth soccer club, your age group state roster and player passes will be the best official team roster/proof of age. If your players are not registered with a soccer club, or you do not have access to the state roster and/or player passes, please provide alternate evidence of your players' Dates of Birth. You may otherwise provide a combination of the two means of proof if you have some players registered with a club, while others are not or a club's state roster and passes are unavailable.
2. A completed copy of the official Tournament Waiver Form (download below).
An official Tournament Waiver Form must be completed for each player on your roster.
Check-In will take place at the tournament venue HQ 1 hour prior to a team's first game. Players do not need to be present.
