Directions: From I-70, take I-675 South to Exit 17
Fairfield Rd. Turn right on N. Fairfield then left onto Col Glenn Highway, then
left on Center Park Boulevard.
From I-675 North, exit 17: turn left on North Fairfield,
then left onto Col Glenn Highway, then left on Center Park Boulevard.

Failure to have all the forms, properly filled out, could
result in your Player, Players and/or your team not being allowed to
participate.
