For those teams wishing to check in their team in person, it will be held on
- Friday, August 15 from 6:00-8:00 PM at the Sicard Hollow Athletic Complex Meeting Room

**GPS NOTES: Depending on your GPS, both addresses could be under Birmingham and/or Vestavia Hills.
The following items are needed:
- 2 copies of your OFFICIAL STATE ISSUED ROSTER
- Copies of your TOURNAMENT GAME CARDS - Issued by SIC
- If you have 3 games, 3 game cards to be stamped
- If you have a possible 4 games, 4 game cards to be stamped
- Official Laminated State Player Passes for All Players
- Medical Release Forms for each Individual Player (for review)
- TOURNAMENT FIELD USAGE LIABILITY WAIVER FORM
- Intent to Travel / Permission to Travel (Out of State Teams)*
- Guest Player Forms* (There is NO tournament GP Form - If your state requires one, then you must fill one out with your State Association, otherwise, State Issued Rosters with Guest Players added will suffice)
*Do not apply to US Club Teams
