Check-In Documentation
Each team must provide:
1. ONE (1) copy of its
official state roster, listing players' names, DOB, and state registration ID
#s. Academy team managers, please note that it is one copy per team entered,
so if your age group is entering three teams, you'll need three copies. We will
keep one copy per team as the official record of your team roster.
2. FIVE(5) copies of the tournament roster/game card, listing all the players playing in the tournament. Once
stamped, one copy will be retained, while four copies will be returned to the
team manager. The team manager will be responsible for handing one copy to the
referees prior to each game (three games, plus final if the team qualifies).
These cards will be used by the referees as official game cards, and will then
be given to the field marshals once the game is completed to act as the
official record of the game. See below for instructions on printing these out.
3. Laminated state-issued player pass (13/14 or 14/15 passes will be accepted and a club Medical Release Form for each player on the roster.
4. ONE (1) completed copy of
the official Tophat Puma Cup Tournament Waiver Form (either downloaded from the
link below, or printed out from your own SIC team page linked to our
tournament).
5. Out-of-State teams will need to provide a completed copy of their state association's 'Notification of Travel' form (or similar).
