Completed and signed waivers will be required at check-in. Players will not be allowed to participate unless these waivers are presented at check-in.

Document | Team RosterPlease print roster. Official roster must be complete and presented at time of check-in. All participating players listed must have full name and date of birth. Any players not registered with Charlotte Independence for the 2021-2022 season may be required to produce proof of age (which could be copy of birth certificate, valid driver's license, or passport).
