- 2014/2015 Official Roster (2 Copies): Either USYS State Association or US Club Soccer
- Guest Player Roster. Write or print guest players on front or back of COPIES of your USYS State Association or US Club Soccer roster.

- Tournament Medical Release Form (Original & 1 copy): CLICK HERE
- Permission to Travel Form for From Your State Association. All teams from South Carolina and teams using US Club Soccer Cards do not need a permission to travel form.
