Online Check-in Requirements

What you will need:

• A copy of your official USYS or US Club Roster
  • if you have guest players, you’ll need to provide (upload) a guest player form from your state association
    • US Club teams will need the US Club Player Loan form and it must be completed by BOTH teams.
  • If you have Club Pass players, you’ll need to provide (upload) a copy of the Club Pass player’s roster
• Player passes (Except WYSA teams)
  • You must provide (upload) current player passes
  • WYSA teams will upload their roster (with photos) to this task
  • Player Passes are not required for U9 and U10
• Travel Permit
  • All out-of-state USYS teams must provide (upload) a valid permit with the name of the event on it
• Medical Releases
  • Medical releases must be current USYS teams, physically signed and dated within 12 months of the event.
  • US Club teams, physically signed and dated within five years of the event.
  • A blank, generic medical release form is provided in the support files of the OnLine Check-in
