Open Document	Medical Form Coach must have in their possesion at all WCOSA events. Does not have to be notarized. No one may see the forms except coaches, club directors and medical personnel. Must be destroyed at the end of the season
Open Document	Injury Report Must be submitted to the league president within 24 hours whenever advanced medical care is given.
