Check In Requirements

 

USYSA Required Documents at Check-in

 

1. Roster: Each player must be properly rostered with the team they are playing on as evidenced by a current Team Roster, Guest Player Roster, Tournament Team Roster or Permission to Travel Roster signed by their appropriate state officials.  
 

2. Player Pass: Players must have current Player Passes.

 

3. Travel Permit: Each Out-of-State team must have a Travel Permit issued by the respective state association, if required. SOUTH CAROLINA teams do not need Permission to Travel; all other teams, including GEORGIA teams do need a Travel Permit.

4. Medical Release: Each player must have his or her appropriate association's medical release.

5.  Tournament Waiver:  Each parent must sign the waiver or electronically sign the waiver during online check in. 

 

US Club Soccer Required Documents at Check-in

 

1. Roster: Each player must be properly rostered with the team they are playing on as evidenced by a current US Club Soccer Team Roster, Guest Player Roster or Tournament Team roster.

2. Player Pass: Players must have current US Club Soccer Player Passes.

3. Medical Release: Each player must have his or her appropriate association's medical release.

4.  Tournament Waiver:  Each parent must sign the waiver or electronically sign the waiver during online check in.
