| Official State or US Club Soccer RosterThis is the roster that you are given by your state/club association.
Guest players and club pass players' names must be handwritten on this roster.

| 2022-23 Player Passes Player passes must include player's name, player ID and picture and must be laminated.

| Permission/Intent to Travel
Permission/Intent to Travel form if required by your State organization.
This is not required of US Club soccer teams.

Document | Player Medical Release Forms Medical Release forms are NOT required to be uploaded. The team must affirm during the Online Check-In process that these forms will be with the team at all times. If your club does not have it's own Medical Release Form feel free to use this basic form. To reiterate, these forms are not required to be uploaded but just to have in your packet at the fields in case a player needs medical attention and no parent is available.
