Check-In should be submitted no later than 8/8/2024
 
Event Check-in for the playdates is an easy one time task. Prior to the first playdate, your team will need to fill out the check-in form below. All that is required in the form is a filled-out event waiver which can be found below.
 
An official roster should be attached to the waiver. Only the manager needs to sign on behalf of all parents/guardians of rostered players.
 
After you have submitted your waiver through the form you are good to go, you are able to just show up and play! If you add players to your roster throughout the season please email gayle@snapsoccer.com
 
EVERY TEAM PARTICIPATING WILL NEED TO FILL OUT THE BELOW WAIVER AND SUBMIT IT USING THE CHECK-IN FORM.
