All Registered Teams:

Team Check-in will be held on Friday August 5th from 6:00-8:30PM at Grimes Bridge Park, 830 Grimes Bridge Road, Roswell, GA 30075.  EVERY TEAM MUST CHECK IN AND BRING THE FOLLOWING DOCUMENTS TO CHECK-IN:

- Team Roster/Waiver Form (Found under Documents on this website)

- GYSA Fall 2011 State Roster (All players must be registered through their club for the Fall 2011 Season)

- Player Card for each Player from 2010/2011 Season or current 2011/2012 Season

- Medical Treatment Form for each player (you should have one through your current team/club)
