Medical Release/Liability Waiver

All participants must have a signed medical release/liability waiver. Primary team contact is responsible for having them at team check in. Please put team name and primary contact person's name on each medical release or staple all medical releases together.  

Required Documents and Equipment

* Team contact must bring 2 copies of your team's roster and all medical release forms of players to check in tent

* Each player MUST have a medical release form to participate. This must be signed by the parents. The form is on the tournament website under "Documents". Your club's medical release form does not apply to this event.

* Each team must bring two different color jerseys. The Home team (the one listed first on the schedule) will wear their light colored shirt and the Visitors will wear their darks. Numbers are not necessary.

* The Home team will provide the game ball for that match.

* Have your team ready to start at the assigned game time. The schedule is tight and we will not have delays due to late arriving teams.

* Each player must wear proper equipment. Shinguards are required.

* If you plan on using the Battleship facilities between games, you may not wear soccer cleats inside their premises. Please plan on bringing other footwear.
