Items needed for check-in · Release of liability (1 copy) - located in the check-in section (coming soon) · Official State roster (1 copy) - 2013/2014 · Player passes - 2013-2014 valid player passes are acceptable. · Copies of birth certificates are required in lieu of player passes · Medical release forms - for review only · Permission to travel/intent to travel for out of state teams (1 copy) · Two copies of each game card for each game. (Details will be emailed where to print Guest Player Form, if needed
