All teams must present:

• signed LSA approved medical release forms;
• an official roster from their home association; and
• additionally, competitive teams must provide laminated player cards for all players and coaches. 

      NOTE:  Any team playing an unregistered or otherwise ineligible player will automatically forfeit all games with no    

                  refund. 

      NOTE:  Any team from outside Louisiana must furnish a travel authorization form from their state association.
