Team Check In Information

Team Check-In for the Bayou Shootout Tournament will be one hour prior to first game at the registration tent at The Lakes Soccer Complex. The following is required for all teams:

1. Player Passes. Player passes will be checked against the Player Roster.

2. Two copies of your current State Player Roster or US Club Soccer Roster. Add player information on any guest players to this copy. Please have the original available in case of discrepancies.

3. Medical Release Forms. Please bring copies of your Medical Release forms for us to keep.  They will be checked against the Player Roster/Release Form. There must be a medical release form for each player listed, except for US Club Soccer, which is made a part of other documentation.

4. Permission to Travel Form.  Permission to travel is required for all out-of-state teams, except for US Club Soccer registered teams. Region III teams only need a Notification of Travel form.
