Team Check-In

Will be on Saturday morning 11/12 from 8am to 9am @ the YMCA Powhatan Fields.  We look forward to seeing you there!

Each team must send a representative to the Team Check-In event.  

JUSA Challenge Cup Tournament Documentation needed at Check-In:

IMPORTANT: JOHNSTON UNITED PURCHASES SUPPLEMENTAL INSURANCE TO COVER NON US CLUB CARDED TEAMS.  THIS ALLOWS EVERYONE TO PARTICIPATE IN OUR UNRESTRICTED US CLUB SANCTIONED TOURNAMENT!

Non US Club Teams:

1. We will need a copy of your team roster including gues players (please see rules for roster / guest player limitations) entered on the JUSA Challenge Cup Roster Form located under the documents tab.  You must email this form to registrar@jusasoccer.org NO LATER THAN WEDNESDAY 11/9.

2. We need proof of age via a birth certificate, passport or other state issued ID.  If you do not keep a copy of any of the aforementioned items, please contact your State Association and request a "Team Information Form".  This must be submitted via email to registrar@jusasoccer.org with the JUSA Challenge Cup Roster Form.

3. A signed US Club Medical Waiver located under the documents tab.  Please do NOT email this documentation.  This must be kept with the team manager during the tournament.

US Club Teams:

 

  1. 2016-17 Coach and Player passes with photo (laminated)
  2. Official Club Roster (original + FIVE Copies)
  3. Completed and signed Medical Waiver for every player.
  4. Guest players:
    1. Must have a US Club Player Loan form. (Guest player will not be permitted to play without properly executed Loan Form). Completed form + FIVE Copies
    2. 2016-17 Laminated US Club Player pass with photo
    3. Completed and signed US Club Medical waiver for every guest player
