• Proof of Birth (for players and guest players) (including guest players)
  • Passcards issues by State Associations cannot be accepted.
  • Please provide one of the following alternate Proof of Birth options:
    • NC Team Information Form which may be obtained by contacting your State Association and requesting a "Team Information Form" for non-USYS tournament
    • SC teams are able to use their team information sheets (Image is at bottom of page).
    • All other state associations contact your club registrar for proof of birth.
    • Or a birth certificate
    • Or a valid Passport
    • Or other government issued ID showing date of birth for each player.

  • Medical Waivers are not required for online check-in, but MUST be present at the fields for each player throughout the duration of the tournament.
  • Medical waivers from USYS, NCYSA, SAY, etc. cannot be accepted. 

No travel permits are needed.
