All teams must complete the mandatory Online Check In. There is no onsite check in.

Official League Roster - All Teams; You receive this from your League or Club

Tournament Roster - All Teams (This is different from your state roster. This roster will be filled out only with the players attending this event.)

Risk Management - All teams (One form per  team)

Player Passes  - All Teams; Please make a copy of multiple passes and upload in as few pages as possible.  Passes for each team  roster must be issued by the same sanctioning body (IE: All USYSA or US Clubs)

ILLINOIS Medical Release - Required for all  non-IYSA teams.  Sanctioning rules require the IYSA Release and will NOT accept any other medical release form. (One per player, please scan all forms into one document to upload)

Form NT  - Required for all NON - UYSA Teams.  Including US Clubs, Triple A and Recreational Teams

Travel Permit - Out of State USYSA teams only.  Complete the request and submit the pdf form 

Guest Player Form - Players from within the same club do not need a guest player form.  Complete the request and submit the pdf form
