Event Check-in for the playdates is an easy one-time task. Prior to the first playdate, your team will need to fill out the check-in form below. Each participating team MUST submit the below waiver via the Check-In form.

Required Documents

1. Snap Soccer Team Waiver - Team name listed on this waiver should match that which is listed on the Roster.
2. Team Roster - You may use an Excel file with player names and DOB if an Official Roster from the US Club or USYS is not available.
