All teams are required to do On-line Check in this year.  


Date to complete On-Online Check In:  February 28th at 3pm (central time)

Requirements for Check-in:
  1. Application Paid in Full – either by Credit Card or Check
  2. Team Conflicts & Constraints (Optional) – entered into the system to make sure we see your coaching conflicts!
  3. Team Contact – who we get in touch with should we need to reach you
  4. Tournament Roster – A state or USSF affiliated league approved roster must be presented at the credentials check.  A maximum of three (3) guest players are allowed. You must have your states approval for guest players and the correct documentation (approved guest player form from your state association).
  5. Guest players – maximum of 3 per team
  6. Player passes Each player must have Player Pass with picture.  All player passes must be checked against the official team roster during the credentials check-in. If you do not have a player pass for a player, they cannot play in the tournament. Guest player passes must be included.
  7. Athlete Waiver – releases tournament from liability
  8. Medical Releases – One medical release form for each player on the form provided by your league or state associationmust be completed, signed by the parent/guardian and, if applicable, notarized.  These are to be presented at credentials check, and all the forms must be presented at the same time.   These forms will be checked and returned to your coach or manager for use in case of a medical emergency. (Within online check in, you can upload one file with all medical releases- just select that there is more than one athlete on the file and then check the athletes that are included)
  9. Out of State Travel Permit  - for teams coming from out of state who require travel permits for State Insurance.
  10. Hotel Reservations – Hotel name, location, phone number, number of rooms and nights.
  11.  Game Cards – the tournament will print these for you!
