Tournament Check-In 
 
 
All teams are required to complete Online Check-in no later than 6pm on Sunday, October 27 by submitting the paperwork described in below chart to TournamentCheckIn@WakeFC.com   There is no fee for online check-in.  Tournament check-in for all teams will be conducted electronically.  
 
 
PLEASE NOTE:  There are different forms for US Club Soccer Carded teams versus NCYSA or USYS carded teams.  All teams are welcomed at our tournament, however please follow the guidelines that are most appropriate for your team in the below chart.
 
 
Once Wake FC Fall Invitational Tournament Registrars have approved your team's roster, and not prior to October 7, you will receive via email a "stamped approved" tournament roster.  Please print 4 copies of the stamped approved Wake FC Fall Invitational Roster and bring to each game during the tournament.
 
 
In the subject line of the email, include your COMPLETE TEAM NAMEAGE GROUP and GENDER of the team.  For example: 16U Boys Wake FC Strikers

 


"US CLUB SOCCER" CARDED TEAMS

US Club Soccer carded teams will submit the paperwork described below to  TournamentCheckIn@WakeFC.com no later than 6pm on Sunday, October 27

 

  • US CLUB SOCCER roster, including guest players.  
  • US CLUB SOCCER passcards for PLAYERS & COACHES.
  • Only the front side that shows picture & date of birth needs to be scanned/emailed
  • You must include the coach's cards
  • TOURNAMENT CONTACT FORM
  • This form is found at bottom of screen.
  • US CLUB SOCCER Medical Waiver (please do not submit during online check-in)
  • Medical Waivers are not required for online check-in, but MUST be present at the fields for each player throughout the duration of the tournament.

 


TEAMS that ARE NOT carded with "US CLUB SOCCER"

(If your team is USYS or NCYSA carded, please follow below instructions)

 

 

Teams that ARE NOT carded with "US Club Soccer" will submit the paperwork described below to  TournamentCheckIn@WakeFC.com no later than 6pm on Sunday, October 27  An example of a team that may not be carded with "US Club Soccer" is a team carded with USYS or NCYSA or SAY or VYSA.

  

  • TEAM ROSTER FORM, including guest players
  • This form is found at bottom of screen. 
  • We CAN NOT ACCEPT rosters issued by State Associations (i.e, we CAN NOT ACCEPT an NCYSA roster.  Please complete the roster form provided at bottom of this screen in lieu of sending us your NCYSA roster).
  • PROOF OF BIRTH FOR PLAYERS, including guest players
  • We CAN NOT ACCEPT passcards issued by State Associations.  In lieu of submitting electronic images of your USYS, NCYSA, SAY, etc cards, please provide one of the following alternate Proof of Birth options for each player:
  • "Team Information Form" which may be obtained by contacting your State Association & requesting a "Team Information Form."  Or you may submit Birth Certificate or Passport or other Government Issued ID showing date of birth for each player.
  • TOURNAMENT CONTACT FORM
  • This form is found at bottom of screen.
  • MEDICAL AUTHORIZATION FORM (please do not submit during online check-in)
  • This form is found at bottom of screen.  Medical Waivers are not required for online check-in, but MUST be present at the fields for each player throughout the duration of the tournament.  We CAN NOT ACCEPT medical waivers from USYS, NCYSA, SAY, etc.  Please use the Medical Waiver provided at bottom of this screen.
 

** Players may NOT be added or written on roster after tournament officials have stamped the team roster approved. **


INSURANCE INFORMATION


 

Please note that the Wake FC Fall Invitational Tournament has purchased supplemental insurance to cover every team not currently carded by US Club Soccer. This additional tournament insurance allows ALL teams to participate in our event regardless of their affiliation.

Medical Authorization Form

This form is to be completed by each player on any team that does not have US Club Cards/Rosters.  Do not email these forms during tournament check-in but Team Manager should have in their possession during the tournament a completed form from each player.
