Fees are $625 for 7-a-side 8U/9U/10U age groups, $625 for 11U/12U 9-a-side age groups, $675 for 12U, 13U, 14U, $700 for 15U - 11-a-side age groups.

1) 2 Copies of Official State Roster (1 will be kept by Tournament Staff)
2) Player Passes
3) Medical Release Forms
4) Tournament Liability Waiver Signed by each parent
5) Out of State Travel Permit (If Applicable)
6) 5 Printed Copies of Tournament Game Cards (will be approved/signed by tournament staff)
