All 2019 LNSC NIKE CUP Participating Teams -

Each team will be expected to present the following information:

Official State Stamped Roster with 4 copies (USYS, US Club or AYSO) - 5 in total
Official State Stamped Guest Player form with 4 copies (if applicable) - 5 in total
Official Player Cards
State Medical Release documentation for each rostered player
Permission to Travel (if out of state)
